CLA-13 - support having a single <ChannelRouting/> element

* Add public methods `GovTalk::setChannelRoute()` and `GovTalk::setAutoAppendOwnChannelRouting()`
* Stop automatically including an extra library-level channel element
  when using the new setter, and document that this is what
  HMRC expect
* Minor refactors
