Major fix to cursor positioning

1. Fixed a bug in double-height line rendering. If the second row of
   the pair also contained the double height escape code, processing of a new pair was started and the next
   normal line was ignored
2. Refactored the code a little to make it less monolithic
3. Added --help and --version command line options
4. CTRL-b can now be used to toggle between bolder and normal colours
5. CTRL-f can now be used to save the current frame to file. The --file option can be used
   to display it
6. When the --menu option is used, the menu displayed is now narrower to better fit 40 column terminals
