Bugfix: a database table was not properly created when doing a fresh install on 0.0.24 and 1.0.1.
Added an option to control whether to update order status when booking an MFB shipment.
Better rendering of shipping services details.

NOTE: this is for Prestashop 1.5 and 1.6 ONLY! See separate release for Prestashop 1.7.
