this feels incomplete, since it is using deprecated ATOM_REF and not utf8 atoms... FWIW, it does work.

Hi,
Here's a scratch PR for supporting NEW_PID_EXT, which has been mandatory for a while now.
Not intended to be merged as-is (even though it does work), I'm mostly fixing the low-hanging fruit and addressing whatever iex complains about next.
