support for enquiry headers

New release based on github.
If enquiry response contains 1-column rows at the beginning of data and number of columns by column names is more then 1, then it's assumed that those ones are "enquiry headers" and added as @HEADERXX columns into resultset into each row. XX is a 1-based number of header.
Improved column names parsing.
