One design tweak to consider: instead of using
MenuDisable to figure out which item was selected, you
could also patch SystemMenu, which is called to handle

- you don't need your heuristic code for figuring out
when to ignore the contents of MenuDisable
- MenuDisable is updated by the MDEF, not the Menu
Manager, and if someone wrote a custom MDEF for a system
menu, they might not know or bother to update
MenuDisable. But SystemMenu would still be called with
the proper info.

I thought about this for a while and decided against it.
The reasons:
The more patches you have, the trickier the interaction
between them. Specifically, the most obvious algorithm
for this would be...

but this is bad if the client callback recursively calls
