Family Planning Notes

1. Before a patient be enrolled in Family Planning, there is a need to check if there have been previous records in the past.

2. To qualify for the FP program, the patient must be:
a. 15-49 both for male and female
b. should be married.

3. For males, the only FP method allowed is vasectomy and condom. CHITS is smart enough to filter the methods based on gender.

4. The following forms are recorded only once. But there is a need to update this if necessary
FP Visit 1
FP History
FP Obstetrical Histrory
FP Physical Examination
FP Pelvic Exam

5. The FP Methods form is updated for each new method for the patient. Provided that the patient have been dropped out from the previous method.

6. The FP Chart shall be updated on a per-encounter basis. there should be an active method being used for this. Use the generic Notes of CHITS for this one.
