fix: normalize excision statements

- ensure excision statements are wrapped in dedicated hi elements (and then wrapped in square brackets) and not included in a broader italicized run of text, in order to facilitate analysis of excision statement practices and prepare for enrichment of excision statements as into gap elements
- fix OCR typos in excision statements and similar bracketed insertions
- normalize representation of 1/2 as ½ in excision statements
