[BUG][Code Sets] Prevent saving codes with empty Value and Code system #1692

Describe the bug
For a new codeset, user can save and commit a version with one code with an empty Value and Code system.

To Reproduce
Steps to reproduce the behavior:

Create new code set
Click on 'Add code'
Click on 'Save'

Expected behavior
The save button should be disabled if value and code system fields are empty
