Launcher icons are sometimes messed up when connected to an external display

Steps to reproduce

1. Open some apps.
2. Connect the device to an external display

Expected Results

The icons in the launchers are arranged in the same exact way as when it's on the device (staged mode - bottom to top, windowed mode - top to bottom)

Actual Results

There are times when some icons are replaced by some other app's icon. Clicking on faulty icons doesn't do what it's supposed to do.

Additional Information

It seems to happen more likely where there are apps open not pinned in the launcher. Also, right clicking on the faulty icon seems to revert it back to the correct icon.
