Add Field Notes to the redcap_to_casesdir datadict construction

If the string tuple in operations contains an additional field for Field Notes, it gets silently dropped. I assume this happens somewhere around here..

The test for this is whether under current configuration the demographics datadict entry for siblings_enrolled_yn_2 contains the field note Revised baseline history based on site verification of sibling status at later time point and final sample of n=831".
