Remove old bidder

@PGAMSSP - for now I've disabled pgamssp for PBJS download, added it to the removal list for Prebid Server 2.0 prebid/prebid-server#2764

We don't generally remove bid adapters outside of major releases, so we'll have to wait for PBJS 9.0, so opened prebid/Prebid.js#10504. Closing this issue.

I think I screwed this up. You wanted to remove the 'pgam' bidder, not the 'pgamssp' bidder.

'pgam' is currently an alias of the adtelligent adapter -- should we check with that team to see if that alias should be removed?

Ok - fixed the docs MD files and issues. Question @PGAMSSP - what about the Prebid.js pgammedia bidder - that's still good?
