Redirect build a wrong URL with twice the to value #233

If I add a rule (on version 4.0.3) like

        <condition type="parameter" name="code">.+</condition>
        <from>^.*$</from>
        <to type="redirect" last="true">http://www.domain.com/qr?%{query-string}</to>

the url http://qr.domain.com?code=444 which points to the webapp using only the urlrewrite lib is redirected to

http://www.domain.com?code=444http://www.domain.com?code=444

with the url repeated twice. The same rule with urlrewrite 3.2.0 is working.

I'm wrong in the configuration or is it a bug?
