I couldn't get the automatic database download to work, when trying to generate the terrain it told me that some river tile was missing. I solved it by selecting a manual GSHGG Path in the plugin settings, downloading the dataset myself, extracting it and creating a touchFile. I'm not sure if I needed to do all those steps, for example the touchFile, but I looked in the source and tried to do the same things manually.
Error output (approximately): Could not run algorithm, couldn't find INPUT: WDBII_shp/f/WDBII_river_f_L01.shp
QGIS: 3.4.15-Madeira
OS: Windows 10 Pro N version 1909 build 18363.1379
Plugin installed via downloading the zip file and installing from QGIS.

@OlaRune I suggest using the plugin with Linux OS, works like a charm.
My Windows desktop is more powerful than my Linux laptop, so I decided to use it on Windows. Maybe it isn't that much of a resource hog and the laptop would be enough.
@erikquinn This is not really a "help wanted", since I've already found a workaround. It is more of a bug report / workaround, either for someone to fix or for others to find so they know how to proceed.
Unless I misunderstood, the issue is the fact that the automatic tile download process isn't working properly. The workaround is good information, but it would be good to just fix the auto-downloader thingy. And I don't know how, so stuck a "help us!" label for anyone who knows how to fix such things. 😄
