Rednukem and NBlood seem to fill to whatever dimensions the window are regardless of the 'widescreen' option being set. Is there no way to play in 4:3 on a widescreen monitor?

Letterbox/pillarbox settings are not available for NBlood - it will always try to fit the game to window size. The best solution is to run in fullscreen with a 4:3 resolution and have ratio scaling set for your graphics card/monitor.

That's too bad - I've been using Gamescope for now to force an 800x600 output, but while playing I'll start randomly losing keyboard input, other times it will be delayed. I don't know what that's about, but as a user of Debian I'm stuck with an old build. It seems to do the same thing with eDuke32.

Ironically, Raze allows pillarboxing, but they don't have the software renderer, which I like for it's classic feel.
