Image Issues (both Image Media and Image Upload)

Hi there,
I encountered an error this evening. I have tried updating, rolling back, etc as well as turning off all plugins to see if there was a conflict. I've attached a screenshot of an error I get when I try to create a new field. For existing media fields, when I click on "set image", the same window is used and I am unable to insert into the post.

Please advise. Thank you!
