I have the same issue with the "Preview" app which is reproducible on my machine:

  1. Start AltTab
  2. Open "Preview"
  3. Close all currently open PDFs
  4. Open several PDFs at once

Then, not all PDF Windows are shown by AltTab.
Sonoma 14.4, AltTab Version 6.67.0
