Improve .NET thread correlation

After looking at some real world application traces, we seem to have an issue with .NET client calls to both downstream services and SQL. We don't correlate the server to client calls correctly. I think this might be similar issue to what we experience with Java and thread pools, so we need to investigate what can be done for .NET.
