ICD-10 Explorer (shiny app)

Create a shiny app to assist with browsing and searching the package data.

App should link to the official DIMDI/BfArM/WHO pages that contain additional metadata (e.g. inclusion/exclusion diagnoses and description) and English translations (where the code is in both versions).

App should be called using a function ICD10gm::explorer() and should be available online (shiny.io?).
