The new version of the subgraph also provides rate providers (now also for gyro pools) and the exemptFromYieldFee flag.

The current subgraph for fantom is still under test https://thegraph.com/hosted-service/subgraph/beethovenxfi/balancer-v2-test?version=pending

Once confirmed that subgraphs look good, this PR should be extended/changed and merged #459
