An index!

I've not been very active here for a couple of weeks for personal reasons, but today I did what I have meant to do for a while: wrote a quick indexing utility indexBook. The result is at Index and the index terms are defined at index6.txt.

Soon, back to improving text -- still waiting for more and more PRs or other contributions!
