"Scheme Namespace" report/form section

As per the latest mockups, there is a section for "Scheme URIs", but per #118 we should actually manage "Scheme Namespace". This can be it's own report/form section but it will be managing a single tile (ie cardinality 1), and a non-localized string for "namespace" (this will require a model change).
