Migrate demographic growth file creation into databricks

Currently, all of the demographic data is managed and extracted by @paulseamer. We need to migrate the raw data to Azure storage, then we can recreate the demographic files by provider as we currently do.

What we can also do then is new cut's of the demographic data, like create a national version of the demographic files
