Allow provider type configuration or workflow

Right now, there are different workflows for specific provider types (like Personal Care Assistant). Those workflows should be separated from the provider types, and given their own names, for which provider types can then be linked to.

The rules surroudning those workflows should be changed to be related to the workflow, and not to the provider type as they are now.
