The only reported issues with installation is caused when you haven't enabled the Notes feature in your org. This is documented as a pre-install step.

The Common Mistake

Install fails and you receive multiple “ActionId specified was invalid for ActionType QuickAction” errors: So far, this issue has only been observed in situations where the Notes feature was not enabled in the org prior to installation per the installation instructions. Please refer to the Prerequisites section (above).
