Phoenix hangs when loading sequences (needs investigation)

Describe the bug

When opening a sequence, I get a console log error for undefined outputFormats and a blank page with the Aerie header. It's not 100% clear whether this is a bug or something I'm missing from my sequence adaptation. I'll open a draft PR that seems to fix the issue, but I'm not Svelte-savvy or Phoenix familiar enough at this point to say for sure.

Reproduction

In Aerie 3.1.0, create a parcel with a sequence adaptation. Create a new sequence with that parcel (I think I also saw this behavior when saving a new sequence, but not certain). Return to the "Sequences" page and then try to edit the sequence.

Firefox on MacOS, local Aerie 3.1.0 deployment. Initially seen on AWS eurc-dev deployment and local Docker deployment, then replicated with dev Vite server.
