Describe the bug
When rendering a repeatable date field in submission, the labels for the field are repeated.

To Reproduce
Create an input form with a repeatable date input and look at it in submission.

Expected behavior
The label shouldn't be repeated and only be displayed once.
Even when multiple inputs are displayed, the label should not be repeated to match other repeatable fields.
