It looks like you are using an SDXL model. The Ultimate upscaler does not work with SDXL models using the default settings.
There is a reddit thread on this topic that goes into details https://www.reddit.com/r/comfyui/comments/17pkqh5/sdxl_ultimate_sd_upscale/
You need much larger tile sizes and padding.
Also, you may need a different control net version ( SDXL vs 1.5 need different versions)
