Why Ultimate SD Upscaler is not working?

I'm getting these poor results that do weird things with the image and I don't understand why if I'm using Scale from image size. Even with denoising strength set to 0 the image changes.

I'd guess the issue is in your model ( what checkpoint model are you trying to use to do the upscaling?) or your img2img settings ( what sampler are you suing, and at how many iterations are you sampling? etc). If you share that info, we might have more to go off of.
