404 Error churchcrm/session/begin

I have tried two new installs, one on a local windows host and one on a hosted Linux environment. Both are responding with a 404 with the URL churchcrm/session/begin. As a heads-up, I did manually insert a row into the version_ver table with the ver_version of 4.5.4 to get around the database upgrade with a blank current version issue. Since there are no records in this newly created table when running install.sql, the line "update version_ver set ver_update_end = now();" does nothing. The log file just has entries stating: ormLogger.INFO: Select * from version_ver order by ver_update_end desc limit 1 [] [].

At his point in time, ChurchCRM is only supported and known to function as designed on Apache web servers. IIS/Nginx/etc are not supported and known to have problems. ChurchCRM currently relies on specific functionality provided by Apache (specifically rewrites via mod_rewrite).
