Warning text does not show up

We have tested the "Warnings" function with survey instruments and have noticed that the text in the "Warning Text" dialog box does not appear when the user selects the text provided in "Answer".
As an example, here is a warning configured for the PHQ-9 survey:

If a user answers "1", "Please call 988" does not show up.
