🐞[Bug]: Bug in appointment form

Describe the bug

Currently the appointment form is accepting the blank form also which is not a good way. We can make certain input compulsory to fill to avoid fake appointments.

Expected behavior

Mark necessary details compulsory to fill before submitting appointment form
